Safety Waiver

ST. LOUIS ART GLASS COMPANY
WAIVER AND SAFETY FORM


All stained glass students are required to complete and sign a liability waiver,
as well as read and agree to abide by the safety procedures
as outlined by St. Louis Art Glass Company.

We will need the following information signed when you arrive for your first class. Please read this waiver before you sign up.

Participating age limit is 16 yrs and up. Students under the age of 18 yrs must be accompanied by a parent or guardian as well as having written approval from the instructor and a parent / legal guardian to enroll in our stained glass classes.


PHYSICAL REQUIREMENTS

Although there can be some sitting during the course, often there will be long periods of standing in order to complete certain tasks. This type of class is not appropriate for people with back, neck or shoulder problems, carpal tunnel syndrome in hands / arms or who cannot stand for periods at a time. Stained glass classes are not suggested for pregnant women.


SAFETY IN THE GLASS STUDIO

~ Always wear safety glasses when cutting / grinding glass and when using equipment.
~ Keep hair tied back.
~ Refrain from wearing loose hanging articles of clothing or jewelry.
~ Wear closed footwear such as running shoes. No flip flops or sandals.
~ Wear long pants during class.
~ Observe safety protocols for each piece of equipment as outlined by the instructor.
~ Instructor must be present while equipment is in use.
~ You may bring a drink in a sealed container, but it must be kept away from your work surface
~ For health & safety reasons, there will be no eating in the classroom.


MISSED CLASSES

Our stained glass curriculum is carefully designed to give you sufficient class time to acquire the skills needed to complete your course.

However, in the event that the instructor needs to cancel a class due to unsafe weather conditions, or other unforeseen circumstances, a “make-up” class will be determined and scheduled by the instructor.

Students will be notified by text &/ or email as soon as possible.

For building your skills and completing your project in time, it’s best if you can commit to the course schedule and not miss any class days. If you must miss a class, or extra time is needed to complete a project once a class session has finished, supervised studio time is available at the instructor's discretion for $15 / hr. Extra studio time will be scheduled at a time that is conducive for both instructor and student. Please note that studio time is not always available and is dependent on glass studio and instructor availability.

If classes must be missed due to illness, please chat with the instructor to see if other arrangements can be made.


WAIVER


In connection with my involvement in art glass classes provided by St. Louis Art Glass Company, I hereby release St. Louis Art Glass Company, it’s agents, representatives, successors or assignees, including St. Louis Art Glass Company management and employees, from all liabilities, actions, claims, damages, demands, costs, and expenses which I am now or in the future have against them, arising out of or in any way connected with my participation in art glass classes, including enroute to or from the program or it’s related events.
I understand that the waiver includes, but is not limited to, all injuries to me, including paralysis or death, and / or loss or damages to any personal property.
I am aware that the use of, and proximity to, glass and certain supplies, tools and chemicals associated with stained glass composition may be a hazardous and dangerous activity.
I also understand that this waiver includes, but is not limited to, any claims that are based on my alleged negligence or any other action or inaction of any of the above parties.
By acknowledging I've read this form, I acknowledge I have read and understood the safety issues, waiver and class requirements that have been outlined, and voluntarily agree to be legally bound by these terms.
